Individuals, families, and referral partners may request services by completing our intake form or contacting our office directly. Once an intake is received, it is reviewed promptly, and a case manager is assigned within 24–48 hours. The case manager conducts an assessment, develops a service plan, and coordinates housing placement and supportive services.

For individuals without a Social Security number, we assist with ITIN-related support and documentation navigation.
