About Creative Community Development

How To Request Services?

 

Individuals, families, and referral partners may request services by completing our intake form or contacting our office directly. Once an intake is received, it is reviewed promptly, and a case manager is assigned within 24–48 hours. The case manager conducts an assessment, develops a service plan, and coordinates housing placement and supportive services.

Intake Requirements

  • Valid government-issued ID or Passport
  • Copy of Medi-Cal card
  • Copy of medical insurance card (if applicable)
  • Copy of Social Security card or knowledge of the Social Security number

For individuals without a Social Security number, we assist with ITIN-related support and documentation navigation.
